Enrollment Information and Requirements

In order to enroll at Herrick Avenue Elementary you will need ORIGINALS of the following documents:

• Proof of Age: Birth Certificate, Baptismal Record or Passport
• Proof of Residence: Current utility bill (Gas, Water or Electricity), Bank Statement, or Official Government Mail (Cal Works, Social Security) with the Parent/Guardian's name on it
• Current Immunization Record
• Driver's License or California I.D. with current address
• Restraining Orders and Custody Orders from a Judge or Court (if applicable)
• Transfer Paper Work (If transferring from another LAUSD school)
• All COMPLETED and SIGNED enrollment forms (Be sure to complete both sides of all forms)
